If you are off work for up to seven days, you do not need a sick note from your doctor.
If you have been sick for more than 3 days, your employer may ask you to complete a self- certificate. You can download the form here:
After the first week
If you have been off work for more than a week you will need a sick note from your doctor. You can do this by submitting an e-consult by selecting the ‘administrative help’ option, alternatively you can also submit a written request at the front desk.
Private sick certificates
We can issue a private certificate if your employer, contrary to national guidance, insists on a “sick note” for an illness lasting up to a week. We have to charge a fee for private certificates which your employer should reimburse.
